The State of Michigan does not require a specific form for Construction Stormwater inspections.
They do, however, provide a template that can be used for inspections. The template can be found in the Michigan SESC Manual.
The Michigan DOT also provides a template for stormwater inspections on their projects.
The State of Kansas does not require a specific form for Construction Stormwater inspections and they do not provide a template.

The State of Hawaii does not require a specific form for Construction Stormwater inspections and they do not provide a template.
The Hawaii Department of Transportation does provide a template for a Final Construction Site Inspection Checklist.
The City of Honolulu also provides a Small Projects Inspection Checklist.
The State of Florida does not require a specific form for Construction Stormwater inspections, but they do provide a sample form within their sample Stormwater Pollution Prevention Plan.
The Florida Department of Transportation does require that a specific form be used for all FDOT construction projects.
The State of Colorado does not require a specific form for Construction Stormwater inspections, but they have provided a sample form that may be required by certain municipalities.
The Colorado Department of Transportation (CODOT) has a required form that must be used on all CODOT Construction projects.
The State of Alabama does not require a specific form for use in Construction SWPPP Inspections. The requirements for inspection reports can be found in the Construction General Permit, dated April 1, 2021.
The Alabama Department of Environmental Management does provide a sample template that can be used for all Construction Stormwater inspections.
