The Arkansas Department of Environmental Quality provides a form for use in Construction Stormwater Inspections.

The General Stormwater Permit states that the ADEQ form should be used for all inspections.

The State of Iowa does not require a specific form for Construction Stormwater inspections and they do not provide a template.

New York does provide an example and template for inspections in their Construction Site Inspection And Maintenance Log Book Sample.

The State of North Carolina does not require a specific form for Construction Stormwater Inspections.

North Carolina does have sample Self-Inspection and Monitoring forms available on their website.



NC DOT also provides a form for stormwater inspections on their projects.

The State of North Dakota does not require a specific form for Construction Stormwater Inspections.

The State of Ohio provides a preferred form for use when conducting Construction Stormwater Inspections.

The State of Oklahoma provides a template for Construction Stormwater Inspections.



Oklahoma also provides a SWPPP template and many other resources on their stormwater webpage.

The State of Oregon does not require a specific form for Construction Stormwater Inspections.

Oregon does provide a sample form that can be used.

The State of Pennsylvania requires that a specific form be used for Construction Stormwater Visual Site Inspections.



Pennsylvania also provides a library of other application and notification forms on their website.

The State of Georgia does not require a specific form for Construction Stormwater inspections and does not provide a sample form.

The State of Connecticut does not require a specific form for Construction Stormwater inspections and does not provide a sample form.

The State of Alaska does not require a specific form for Construction Stormwater Inspections and does not provide a sample form.

The State of Colorado does not require a specific form for Construction Stormwater inspections, but they have provided a sample form that may be required by certain municipalities.

The Colorado Department of Transportation (CODOT) has a required form that must be used on all CODOT Construction projects.

The State of California does not require a specific form for Construction Stormwater inspections and does not provide a sample form.

The California Department of Transportation (CALTRANS) DOES require a specific form be used for Stormwater Inspections on CALTRANS construction projects.

The State of Arizona does not require a specific form for Construction Stormwater Inspections.

Arizona has provided a sample inspection form (in three parts) on the Arizona Department of Environmental Quality website.

The State of Alabama does not require a specific form for use in Construction SWPPP Inspections. The requirements for inspection reports can be found in the Construction General Permit, dated April 1, 2021.

The Alabama Department of Environmental Management does provide a sample template that can be used for all Construction Stormwater inspections.
